ANAPHYLAXIS, ASTHMA, DIABETES, & SEIZURES

Anaphylaxis, Asthma, Diabetes, and Seizures are medical conditions that REQUIRE one of the following from a parent/guardian each school year:

• a corresponding Care Plan form (below) on file.

• a signed acknowledgement that a Care Plan is not required for their condition (in PS eforms). 

• Food Allergy & Anaphylaxis Emergency Care Plan
• Asthma Action Care Plan
• Diabetic Medical Management Care Plan Form
• Seizure Action Care Plan
